3. Approved Cleaning Service & Invoice

Enter the Grand Total exactly as shown on the approved itemized PhotoBiz invoice. Individual add-on prices are not entered here. Appointments require company confirmation.

Enter the approved PhotoBiz invoice grand total, then click Calculate / Update Approved Quote. This agreement calculates the 50% deposit and balance; individual add-ons are already included in the invoice and are not charged again.

4. Approved Add-Ons — No Individual Prices

Select only add-ons already included in the approved PhotoBiz invoice. All selected add-ons will appear in the agreement and email record. Additional requests require separate office approval.

Appliance Cleaning

Appliance services cover only the approved selections. Gas, plumbing and electrical disconnections, repairs, and unsafe appliance movement are excluded.

6. Scheduling & Service Hours

Standard Service Hours: Monday–Friday, 8:00 AM–6:00 PM. All appointments require company confirmation.

Same-Day / Emergency Priority Service

$175 minimum or 25% of the approved base price, whichever is greater.

Only the highest applicable scheduling surcharge applies. Fees do not stack. Any applicable fee requires company approval and is not automatically added to the Grand Total.

Customers must provide safe, authorized property access. Requested appointments remain subject to company confirmation.

7. Cancellation & Rescheduling

A 50% booking deposit may be required and is credited toward the approved balance. An applicable cancellation fee may be satisfied using that deposit without double charging. Rescheduling is subject to availability.

8. Property Condition & Scope

Pricing reflects the approved service scope and disclosed property condition. Unexpected heavy buildup, excessive clutter, trash, pet hair or additional work may require a revised quote and customer approval before extra services are performed.

9. Safety & Service Limitations

Las Vegas Pro Cleaners LLC may decline unsafe work involving hazardous materials, unstable surfaces, sharps, unsafe access, unsafe appliance movement or other conditions that create unreasonable safety risks.

Specialty remediation, repairs, hazardous waste handling, heavy hauling and unsafe elevated access are excluded unless expressly approved and safely serviceable.

10. Pre-Existing Conditions & Service Documentation

Customers should disclose fragile, damaged or unstable items. Pre-existing wear, damaged finishes and loose fixtures may be documented.

Relevant service-area photographs may be used for condition verification, quality control and concern resolution. Public marketing use is not automatically authorized.

11. Payment & Additional Services

The customer agrees to the approved PhotoBiz invoice amount shown in this agreement. All approved add-ons are already included in the Grand Total.

Additional services require a separate quote and approval before work or additional charges. This agreement does not create another invoice or collect payment directly.

12. Late Payments & Quality Concerns

Late payment: No fee days 1–3; $25 on days 4–7; an additional $25 beginning day 8, capped at $50, only where permitted by law and disclosed on the approved invoice.

12. Quality Concerns

Customers should promptly report cleaning concerns. Las Vegas Pro Cleaners LLC will review concerns against the approved service scope, property condition and available service documentation.

13. Cleaning Personnel

Las Vegas Pro Cleaners LLC may assign qualified employees or cleaning professionals according to operational needs. Specific personnel cannot be guaranteed for every visit.

I agree to the approved invoice amount and separately authorized charges.

7. Recurring Membership

If applicable, I understand my membership frequency, commitment and approved included add-ons.

Electronic Signature

Typing your full legal name and submitting this form constitutes your electronic signature and acceptance.

Customer email confirmation requires the Web3Forms paid autoresponder feature enabled in your Web3Forms dashboard. The free plan sends the submission to the business, not an automatic customer copy.
